Below is a list of documents for your review and forms to be completed and returned to the school during the first week of classes.

NOTE: Once in-person hybrid learning begins at PBHS, these forms will be necessary.

If you do not have a printer, you may request the forms from your school office.

Student Publicity Restriction Form (Only complete if you DO NOT want your child photographed for district publicity)
