COVID-19 Vaccine/Booster Leave Request Procedure
Obtain the COVID-19 Vaccine/Booster Leave Request Form and Verification of Appointment Form by either printing here or requesting a hard copy from your building’s main office.
- Submit the request form to your immediate supervisor at least 24 hours prior to your appointment.
- Login to your Frontline Absence Management account and mark yourself out as sick. **Once the verification form is received, approved and processed, the Human Resources/Attendance Department will credit back the sick time and award the COVID-19 vaccine time.
- Take the verification form with you to the appointment for the provider to complete.
- Submit the completed verification form and a copy of your vaccination card to the Human Resources/Attendance Department at the district office.
